If your Application ID number does not start with 99, your grant is in our new online system and you can now complete your reports online.
Log in to your portal and then click on the 'My Grant Reports' icon.

If your Application ID number begins with 99, your grant is in our previous system, please use the reporting forms below:
1) Reporting Forms for Multi-Year Grants
Grants that are longer than one year in length- Additional Multi-Year Financial Table
- Additional Multi-Year Outcomes Table
2) Reporting Forms for Capital Grants
Capital (equipment or renovations) grants
If your grant contains funds that have been allocated for other non-capital items as well, use either the Multi-Year or One-Year Forms
3) Reporting Forms for One-Year Grants
Grants that are one year or shorter in length
